Why was a resident sitting on the toilet for 3 hours?

The recent media story about a Middlesbrough care home and a resident sitting on the toilet for 3 hours is alarming. Other problems were lack of care documents and residents left in bed all day. Alarm was the aim of the reporting. I know that Care Quality Commission identified this but how long before it was identified? How can this be identified earlier without micro managing? Surely those within the care homes need to be confident enough to speak up. Do staff realise these are examples of abuse? There has to be a better way.
